Please see attached poster regarding this valuable service which provides support from volunteers to people over 60 years of age, working closely with the Intermediate Care team and Remote monitoring hub who provide urgent care for acutely unwell patients to avoid admittance to hospital. Volunteers support with welfare checks, helping people up following a fall, collecting emergency prescriptions, providing reassurance while waiting for family, friends, primary and secondary care services to arrive, doing small amounts of shopping and topping up gas and electricity meters.
